Referral Process

Agency Referrals: Charities, agencies, and third-party organisations (such as mental health, justice, and drug and alcohol services) must complete all sections of the referral form, including the grey areas. Incomplete forms may result in processing delays or the referral being rejected.

Self-Referrals: If you are referring yourself, please complete only the white sections of the form.
Please Note: GRACE is funded to provide peer-led support and pathways to social inclusion. Therefore, anyone attending our services must be able to participate independently. If you have any further questions please feel free to get in contact here
